This very short quote is a good reminder that effective root cause analysis needs to identify the fundamental reason(s) an issue occurred, not place blame or just default to the ubiquitous 'human error.' And the word "remedy" is a great metaphor for developing and implementing effective corrective and preventive actions that 'cure,' not just bandage, those true root cause(s).
